Program consisting of a system
of coordinated health care
interventions,
communications and
education. The program
is intended for people living
with chronic illness and
conditions in which patient
self-care efforts offer a viable
opportunity to improve the
quality of life and maintain health.
The Goals of Disease Management are:
 
  1. •    Help avoid costly and debilitating complications of
         improperly treated diseases
  1. •   Assure that patients obtain the proper medical care
         needed to reduce health risks
  1. •   Adhere to your physician’s plans of care
  2. •   Promote self management to improve long term    
         outcomes and reduce avoidable hospitalization
         and emergency care visits
  1. •   Provide the patients with the education,
         communication and intervention necessary to
         learn proper self management techniques
